Getting started

Begin with a brief inquiry. Sensitive documents move through the secure client portal after CFV creates your account.

A service is not reserved until CFV sends written confirmation and required documents, consents, and payment are received.

Prepare for intake

Documents and information CFV may request

  • Complete, legible, file-stamped current court orders
  • Parenting plan or residential schedule
  • Protection orders, no-contact orders, dependency orders, or criminal orders affecting contact
  • Later modifications and written referral requirements
  • Contact information and a safe method for CFV to communicate
  • Known allergies, medications, accommodations, and safety concerns
  • Requested dates, service level, location, transportation, and approved participants
Do not attach records to a public inquiry. CFV will send portal instructions after reviewing your request. The public form is not an emergency channel and should not contain a child's full name, confidential address, case narrative, medical record, or court document.
